Submission of any form does not guarantee reservation or placement. Confirmation for any reservation must be made by a Nampa Recreation Center or Nampa Parks and Recreation Department representative.

- Cardio Area Etiquette (PDF) - Guidelines have been established for the safety and enjoyment of all our participants.
- Exercise Spotlight Video Web Instructions (PDF) - Instructions to access the library or exercise videos on YouTube.
- Health History Questionnaire (PDF) - A Health History Questionnaire needs to be completed when scheduling an appointment with a Fitness Trainer.
- Medical Clearance (PDF) - Based on American College of Sports Medicine guidelines, a medical clearance may be required prior to meeting with a Fitness Trainer.
- Power Plate Contraindication Form (PDF) - A Power Plate Contraindication Form needs to be completed when scheduling a vibration training session with a Fitness Trainer.
- Virtual Fitness Consultation (PDF) - Print this form to discuss with the trainer during your Virtual Fitness Consultation appointment.

- Youth Health History Questionnaire and Pledge Forms (PDF) - Complete a Youth Health History Questionnaire and Pledge Form prior to meeting with a Fitness Trainer for an appointment.

- School Group Request Form (PDF) - This form is required for school groups to visit the Harward Recreation Center for organized activities and a discounted rate! Please print this form and return it to the Harward Recreation Center to confirm.
- 2022 City Licensed Tree Companies (PDF) - A list of companies that are licensed with the City of Nampa to trim trees within the city. They follow proper pruning methods that provide for healthier and stronger trees in the community.

- Parks Seasonal Work Interest Form (PDF) - Forms are for seasonal staff positions with the Nampa Parks Department Forms can be filled out and turned in at the Park Office during regular work hours or e-mailed to Parks@cityofnampa.us
- Service Projects Proposal Form (PDF) - Complete this form and submit to the Park Maintenance Office. For more information call 208-468-5890.
- Treasure Valley Tree Selection Guide (PDF) - The Treasure Valley Tree Selection Guide was created by public, private and non-profit individuals and groups dedicated to the healthy development of the Treasure Valley's urban forest. The guide was developed to answer common questions by both home and business owners regarding the appropriate tree selection for local landscapes.
- Right-of-Way Tree Permit - Forester approval required on all right-of-way tree work, including planting, for both business and residential. Please fill out the Right-of-Way Tree permit form and turn into Forester for approval. Forms can be mailed to 312 1st Street S Nampa, ID 83651, emailed to mancinia@cityofnampa.us or faxed to 208 465-2321. There is no cost for the permit. Note: A permit is required when working with the trees along the right-of-ways within the city of Nampa per City Code 5-2-4.
- Adopt-a-Pathway Program - A great way for families, organized groups, community service groups and businesses to take an active role in helping to maintain our city pathways. Program participants can adopt a section of pathway for a set period. Adoption would include regularly monitoring the are for debris, trash, invasive species, vandalism, as well as help with other maintenance needs. This is a great opportunity to make a difference in the local community, build relationships and enjoy the outdoors at the same time.
- Nampa's Urban Forestry Management Plan - Take a minute to review the vision and mission of the city's Forestry division and what that means for the urban forest of Nampa.
- Memorial Tree & Bench Program – This program is a great way to commemorate and pay tribute to a loved one, pet, special occasion or event.
- Preschool Registration Form (PDF) - This form will need to be submitted upon initial registration for the preschool program. This form only needs to be submitted once.
- Rock Wall Waiver (PDF) - The Harward Recreation Center Rock Climbing Wall Waiver is required for all participants prior to using the Climbing Wall. Participants under the age of 18 must have a parent or legal guardian's signature on this form before climbing.
